Medications
- Medications will only be administered after parent/guardian delivers the completed Medication Permission form signed by both the parent/guardian AND physician. This form must be provided for prescription medications, over the counter (OTC) medications given outside of the manufacturer’s recommendations, herbal and food supplements, alternative medicinal products, and other items that do not have FDA approval.
- Over-the-counter (OTC) medications only require a parent/guardian signature on the Medication Permission form. OTC medications can only be given based on the manufacturer recommendations on the bottle.
- Parent/guardian must deliver medications in the original labeled container. Many pharmacies will give extra medication containers for school at your request. Students may not deliver medications.
- A new Medication Permission form is required after July 1st for each school year.
- Ascent Academy does not provide OTC medications such as Motrin, Tylenol, or cough drops.
Immunizations
- All students are required by South Carolina law to provide up to date vaccine certificates by the first day of school. Please submit a copy to the nurse and keep the original for your records.
- Students who do not meet immunization requirements will be excluded from school.

When to Stay Home from School
- Fever of 100 degrees or higher. Student should be fever free for 24 hours before returning to school.
- Students should not be given Tylenol (acetaminophen) or Advil (ibuprofen) to reduce fever and then sent to school. Student may still be contagious to others.
- Nausea or vomiting within the last 12 hours
- Severe headache
- Diarrhea within the last 12 hours
- Red, watery eyes with yellow drainage
- Unexplained rash
